Change of Contact Details
Students need to change their contact details (address and phone number) via My Student Admin (you will need your student ID number and UTS email password).
Please also ensure that you inform the Student Officer: Research and your supervisors of any changes to your contact details.

Use this form to change your attendance pattern, your thesis title, your supervisor or co-supervisor.

Application for Leave of Absence
Please note that Applications for Leave of Absence will not be considered for that semester if submitted after the HECS census date (dates are on form). Retrospective Leave of Absence cannot be approved. Each research degree student is eligible for 2 semesters of leave during the period of their candidature.

Application for Extension of Candidature
If you are not going to have your thesis ready for submission by the Expected Work Submission date you will need to apply for an extension to your candidature. You need to provide adequate reasons for your inability to complete your work in the prescribed time. If approved by the Faculty and the University an extension of one semester is usually granted.

Transfer from one program to another
If you wish to change from the program that you are currently enrolled in, for example, changing from a Doctor of Nursing to a PhD, upgrading from a Masters to a PhD, or downgrading from a PhD to a Masters you will need to complete an Application for Course Transfer form and satisfy the requirements for the transfer you wish to make. Before completing the application you need to ensure that you have spoken with your supervisors and the co-ordinator of the program that you wish to enter.

Withdrawal from your course
If you are unable to continue in the course in which you are enrolled you will need to formally withdraw. You should not withdraw without discussing possible options with your supervisor and research staff, such as taking leave of absence or transferring to another program. You will not be withdrawn from the course until you have submitted a Notification of Complete Withdrawal form (Student Administration Unit web site).
